Our practice is committed to preventative care. We may issue you with a reminder notice from time to time offering you preventative health services appropriate to your care.

It is also your responsibility to update your details with the practice including Address, Phone contact numbers, Next of kin details when they change so that communication between you and your doctor is not impeded. We will not be able to contact you for follow-up of any abnormal results or reminders if we do not have your correct details.
